Insurance & Payment
Do you accept insurance for sessions?
Yes! Miriam Boutte Nutrition conveniently accepts insurance and provide superbills for out-of-network reimbursement, making quality nutrition services more accessible. 95% of Miriam Boutte Nutrition patients pay $0 out of pocket.
Miriam Boutte Nutrition is in-network with:
– Blue Cross Blue Shield (BCBS/Anthem)
– UnitedHealthcare (UHC)
– United Medical Resources (UMR)
– Aetna
– Cigna
– Medicare (Medicare covers visits for individuals with diabetes or non-dialysis kidney disease.)
How do I know if my insurance will cover this?
95% of Miriam Boutte Nutrition patients pay $0 out of pocket. If you are concerned about coverage, please call your insurance company’s Member Services department. You can also visit the insurance guide page to review helpful questions to ask when verifying your nutrition benefits.
To use insurance for services, your insurance policy must be active at the time of your appointment. Information provided by insurance representatives is not a guarantee of payment.
Miriam Boutte Nutrition / Miriam Boutte follows a Billing Transparency & Insurance Responsibility policy designed to prevent surprise charges. If a claim is denied, you will not be charged for sessions already completed. If cost-sharing requirements such as copayments, coinsurance, or deductibles apply, we will review next steps and self-pay options with you before any future charges are applied.
How are claim denials, copays, coinsurance, and deductibles handled?
95% of Miriam Boutte Nutrition patients pay $0 out of pocket. At Miriam Boutte Nutrition, we are committed to providing a clear and no-surprise billing experience. The following applies only when an active insurance policy is on file at the time of service.
If an insurance claim is denied, you will not be charged for any appointments that have already occurred. Those sessions will be provided at no cost to you.
If your insurance plan includes a copay or coinsurance, you will not be charged more than the initial estimated amount for any sessions completed until your actual responsibility is confirmed. Once your confirmed copay or coinsurance is received from your insurance carrier, you will be notified via email. All future sessions will then be billed at the confirmed amount.
If your insurance applies services toward your deductible, this may not be known until after a claim is processed. In that case, you will be given two options for any sessions that occurred prior to this determination:
• You may choose to pay the deductible and continue receiving services billed through insurance, or
• You may choose not to pay the deductible, in which case you will not be charged for sessions already completed. All future appointments will be billed at the self-pay rate.
If a deductible applies and is not paid, we are unable to continue billing insurance for future services. To remain compliant with insurance billing requirements, insurance claims must be withdrawn or not submitted, and services will transition to self-pay.
If your insurance is inactive, unavailable, or terminated at the time of service, sessions will be billed at the self-pay rate.
A valid payment method is required to be kept on file; however, you will always be notified before any charges are applied.
Are my visits covered even if I have a deductible to meet?
Some insurance plans, including many PPO plans through BCBS/Anthem, Aetna, Cigna, United Healthcare, and UMR, include preventive nutrition counseling benefits that may be covered without requiring you to meet your deductible.
95% of Miriam Boutte Nutrition patients pay $0 out of pocket. Coverage varies by plan and requires active insurance coverage at the time of service. In some cases, deductible responsibility is not known until after a claim is processed by your insurance carrier.
If we later learn that your visits apply toward your deductible, you will not be charged for sessions that already occurred before this determination was made. We will review your options with you before any future appointments, including whether to continue using insurance or transition to self-pay services.
If a deductible applies and is not paid, we are unable to continue billing insurance for future services. To remain compliant with insurance billing requirements, insurance claims must be withdrawn or not submitted, and services will transition to self-pay.
Do you accept Medicare & Medicaid?
Miriam Boutte Nutrition is a preferred provider for Medicare. Medicare covers Medical Nutrition Therapy only for Type 1 or Type 2 Diabetes and non-dialysis kidney disease (stages 3–5).
